It's always the maximum of EVERYTHING.

I'd give examples, but then again, all I need to say is that I'm talking about levels/lives/stats/whatever. Anything that involves numbers, really.

I think the highest your attack power could go up to is 255 in Chrono Trigger if I recall.

The highest Attack you can do (without any armor like the ring of Aeres) in Symphony of the Night is 255.

It's probably because the game designers don't want to have the arrays eat up too much memory.

256 (or 255 if 0 is not the null value) is likely more than enough for statistic value but be low enough not to eat up too much space in the code.
It also would allow for a wide enough listing of enemies (or, using Pokemon as an example, individual Trainers as a reference point for enemy parties).
However, in some cases not all of the slots in an array are used, which results in (as an example) Missingno and the Glitch Pokemon.
