I've heard all kinds of things, from getting shot in the foot and dying from loss of blood, to getting run over by a scooter...
but I have to say, the absolute worst... is the first goomba.

Fork. From the ceilling. On level 100. Of the Pit of 100 Trials. When Bonetail has only 1 HP left.
That actually happened to me. I had just enough HP to survive his attack and kill him and this FORK comes from the top of the stage and KILLS ME! |

Just when you are about to get him.......he some how manages to get you and you have to start form the beginning with this dude.

Losing to Wracktail in the Pit of 100 Trials. Even with about 4 Super Shroom Shakes I still lost. No thanks to his 10 damage attack.
Not to mention how much of a pain it was to make it there. Especially without any movers, like in PM:TTYD. >_< |

When you're fighting the final boss in a Legend of Zelda game and you almost beat the final boss when they make the final boss get tougher all of the sudden, then you die after the boss uses some uber move. |

When you pause the game to go to the bathroom or something. Then something knocks over the controller (i.e. the dog) and unpauses the game. Your character is left standing there like an idiot for enemies to attack. |

Oh God I have a load of stories for this.
Just Recently: Starting a new game of some sort and then dying after playing for a few hours and not saving your file once. That's really bothersome. Just happened to me when playing some GBA Castlevania game (Where you play as Juste) and when restarting on FF3 DS.
Dying at 99% on Bark at the Moon on expert. That redefines rage.
Dying at any point of time in Dirge of Cerberus.
I have a load more, those just instantly come to mind. |

Harmony of Dissonance.
Anyway, just after getting to one of the Bowser fights in SMB, getting hit by a lava bubble. They're so freaking annoying, especially later on when they're right next to Bowser. |
