How do you make desktop icons without converters?

Because icon converters fail and can only make lines. I've converted like a thousand of images with three different converters, and they ALL came out to something similar to "____" ;;
And I've also tried adding .asdf to the end and it didn't work. It just saved as a "img.asdf.png or .jpeg.
And I'm only saying asdf because I forgot what the file type is called. .-.

Seven DIFFERENT ico converters on the internet make them 16 x 16, which is TOO *crag* SMALL. IT COMES OUT ALL *crag* BLOCKY AND SUCKISH. ;;

Most high-end programs allow the user to save in the .ico format (i.e/ Photoshop, Paint Shop Pro). If you can't afford those, then, try something free, like Infranview.
