The Eye-Slime form changes into a floating eyeball, drills into the victim's head and melts it from the brain and then the skull and out until the head is nonexistent and the eyeball can control the body like a meatpuppet. The thing at the very bottom of the page regarding the stomach is a image of how it eats: The meat-puppet's stomach is replaced by a big mouth with tiny teeth and a bunch of tendrils which come out to grab the prey and pull it in to eat it.

These are tentacles or tendrils, not genticles.
And its not Mr. I. Mr. I is a big eyeball. This starts off as a tennis-ball sized eye in slime-like stuff and then becomes a head-sized eyeball with vein-like tentacles coming out of the bottom.
