In the field where it says "Item uses MP?" click the check box.

This is as simple as m'aiq can put it:

For every 5 HP healed, it takes 1 MP.

For every 4 HP taken*, it takes 1 MP.

*roughly for lower numbers, not sure if it continues that way.
